BF Care Health Navigator

The BF Care Health Navigator will work with members of the clinical care team to enhance better health and take the initiative to ensure clinical care and related support services are made available to participants. The BF Care Health Navigator will be the main point of contact for the participant from identification through program completion. They will perform initial intake, scheduling all appointments, scheduling labs and following up on results, and monitor program compliance. While monitoring participants through the platform, the BF Care Health Navigator monitors alerts, and screens participants for medical or social concerns. The Health Navigator schedules and participates in audio-visual calls with participants who require coaching and motivation. They will report abnormal values or other participants concerns to the care team per escalation protocols.


  • Perform initial intake of participant and document accordingly
  • Orient participants to the program including daily tasks, education, and appointment schedule.
  • Set up participant virtual visit schedule for Health Navigator and Advanced practitioner per program protocols
  • Obtain prior authorization for all medications and labs as required
  • Monitor participant compliance with program and intervene as necessary
  • Support participants physical and emotional wellness with a holistic health approach
  • Communicate concerns, changes in condition and all medical complaints to care team per protocols
  • Assist with the troubleshooting of monitoring devices and escalation of technical issues to the Customer Success team
  • Monitor all events and escalate per protocols
  • Answer, direct, and follow up on incoming calls
  • Document patient interactions appropriately in related applications, including but not limited to the Biofourmis EMR, and client EMRs.
  • Ensure a high-level of patient and staff safety by being an active participant in quality and patient safety initiatives.
  • Other duties as assigned



  • High school diploma or GED required. Bachelors in healthcare related field preferred.
  • Prior healthcare experience required
